The code for the safe in the Treatment Center, Third Floor, Room 3E is 7-3-1. You can figure it out by listening to the knocking of the pipes. (The explanation would be that the person that inputed the code heard the pipes knocking all day long in the same patern and picked that combination without realising it, as implied by the document "Knocking in Pipes")
You don't always need to use a rope arrow in the elevator shaft of the Women's Ward. As long as you have purchased the "Leather Padding" upgrade, you can simply drop down the shaft and climb out through the stairs.
The document "Extracting the Primal" counts towards the "BARON NORTHCREST" category in the Journal, not the "CHAPTER 5".
There are a total of 8 Food items in the level, which means that (even if you have purchased the "Troy's Satchel" trinket to expand your inventory to a maximum of 7) there is no way to grab them all in a single run without spending at least one of them.
